About the Role

Carevanta Home Care is a family-feel in-home care agency in the Clackamas/Salem area, and we're looking for a warm, experienced semi retired nurse to be the clinical heart of our team. This position has a highly flexible schedule, specific tasks will be tagged with deadlines. This isn't a bedside hospital job. It's a low-key, meaningful part-time role where you'll use your nursing know-how to guide our caregivers and keep our clients safe and well cared for at home, without the grind of a full-time nursing career. We built this position with a semi-retired nurse in mind: real purpose, real flexibility, no burnout.

What You'll Do

  • Lead client intake by reviewing a new client's health information as part of onboarding — this is a key first touchpoint that flags clinical needs before the client is even through the door

  • Determine whether a client's condition is stable and predictable enough for specific tasks to be safely delegated to caregivers
  • Complete regular reviews of medication and nursing care plans. Some clients will require this once a month, while others quarterly.
  • Lead the clinical portion of new caregiver training, these sessions will be organized in cohorts. Sign off on caregivers' hands-on medication skills after training
  • Delegate specific nursing tasks to trained caregivers and keep the delegation paperwork current
  • Set up the medication plan for clients receiving full medication administration, based on the doctor's orders.
  • Keep medications and doctor's orders organized and up to date, and follow up with the doctor's office whenever something needs confirming or updating
  • Be a phone call away if a caregiver or client needs your clinical judgment.
  • Review change-in-condition reports and decide whether a client's care plan needs updating. And help us recognize when a client's needs have grown beyond what we can safely provide, so we can point them toward the right kind of care
  • Join our small team's quarterly check-in to talk about how we're doing and where we can do better
  • Weigh in on incident reviews and our quarterly quality check-ins when something's medication- or health-related
  • Write a short clinical wrap-up when a client's services end for health reasons
  • Help a client's family understand what's involved if they choose to self-direct medications

What We're Looking For

  • A current, active Registered Nurse license in Oregon, in good standing
  • Have at least 5 years of experience in the last 8 years.
  • A caring, patient teaching style comfortable coaching non-medical caregivers
  • Someone who wants a flexible, mostly self-directed schedule, not a 9-to-5
  • Able to travel occasionally to client homes in the Clackamas/Salem area
  • Semi-retired, scaling back, or just want meaningful part-time work? This role is built for you.
