Sonder Healthcare is seeking a Field LVN to provide high-quality home health care to patients across West Sacramento and Davis, CA. The LVN will deliver direct patient care under the supervision of a Registered Nurse (RN), follow individualized plans of care, provide patient and caregiver education, and collaborate with the interdisciplinary team to support positive patient outcomes. This role involves visiting patients in their homes and ensuring continuity of care.
Qualifications
Graduate of an accredited Practical Nurse or Vocational Nursing program. Minimum of 3 years nursing experience; Home health experience required. Licensed as a LPN or LVN in California. Current BLS certification through American Heart Association Strong verbal and written communication and organizational skills. Valid driver’s license and reliable, insured transportation for home visits. Adheres to professional standards and nursing best practices.
Key Responsibilities
