About this role

Monday–Friday, 8:00 AM – 5:00 PM

  • Contract Length: 15 Weeks
  • 40 Hours per Week

Responsibilities

  • Coordinate patient care across providers and departments
  • Perform clinical triage via phone, EMR, or in person
  • Review labs, imaging, and clinical results
  • Prepare patients for procedures and provide education
  • Manage referrals, authorizations, and follow-up care
  • Document clinical interactions accurately in Epic EMR
  • Collaborate with multidisciplinary care teams

  • Active RN License
  • 2–3+ years of RN experience (3–5+ years preferred)
  • Family Medicine experience preferred
  • Care coordination or case management experience preferred
  • Strong clinical triage, communication, and patient education skills
