Green Health Collective is seeking an *experienced, dependable Medical Assistant* *or LPN *to join our small, single-clinician private practice. This full-time position plays an essential role in supporting high-quality patient care, efficient clinical workflows, accurate documentation, and a positive patient experience. The ideal candidate is proactive, organized, detail-oriented, and comfortable balancing clinical and administrative responsibilities in a fast-paced outpatient setting. *Prior Athena EHR experience is required.* Experience with eClinicalWorks (eCW) or Epic is a plus.

*Clinical Responsibilities*
- Conduct patient intake and rooming procedures.
- Obtain and document vital signs, weight, BMI, medical history, medication reconciliation, and review of systems.
- Complete pre-visit planning, including chart preparation and identification of preventive and chronic care gaps.
- Use preventive care and chronic disease checklists to support comprehensive patient visits.
- Prepare examination rooms and patients for visits, procedures, and diagnostic testing.
- Perform EKGs, point-of-care testing, and basic clinical procedures as delegated.
- Assist the clinician with examinations, procedures, wound care, and other clinical services as necessary.
- Accurately document patient histories, clinical findings, and care activities in Athena.
- Support medication-refill workflows, follow-up care, and chronic care management.
- Reinforce clinician-directed patient education regarding medications, disease prevention, lifestyle changes, and treatment plans.
- Participate in preventive care initiatives, population health activities, and quality-improvement projects.
- Follow patient-safety, infection-control, emergency-response, and BLS protocols.
*Administrative Responsibilities*
- Process prior authorizations for medications, testing, and procedures.
- Coordinate referrals and assist with tracking consultation records and follow-up recommendations.
- Schedule appointments and respond professionally to patient telephone and portal inquiries.
- Maintain accurate and timely documentation in the electronic health record.
- Support diagnosis-code entry and other medical administrative workflows.
- Assist with care coordination and communication among patients, pharmacies, specialists, laboratories, and outside healthcare organizations.
- Help identify opportunities to improve workflow, teamwork, patient satisfaction, patient safety, and visit productivity.
*Required Qualifications*
- *Previous Medical Assistant/outpatient LPN experience* in primary care, internal medicine, or another outpatient clinical setting.
- Experience supporting preventive care, population health, or chronic disease management programs.
- *Hands-on Athena EHR experience/proficiency is required.*
- Proficiency in patient intake, vital-sign measurement, medication reconciliation, and clinical documentation.
- Experience performing EKGs, point-of-care testing, injections, and basic outpatient procedures.
- Working knowledge of medical terminology, anatomy, physiology, medications, and ICD-10 diagnosis coding.
- Experience with prior authorizations and referral coordination.
- Strong computer, documentation, organizational, and time-management skills.
- Excellent communication and interpersonal skills.
- Professionalism, accountability, sound judgment, and close attention to detail.
- Ability to work independently and anticipate the needs of a small clinical practice.
- Commitment to patient-centered, evidence-based, and inclusive care.
- Must provide 3 current references (2 past employer, 1 professional)
*Preferred Qualifications*
- Medical Assistant certification or LPN licensure/certification or other equivalent/relevant healthcare certification.
- Current Basic Life Support certification.
- Experience with eClinicalWorks (eCW) or Epic, preferred.
