Join our dynamic healthcare team as a Registered Nurse specializing in Community Case Management! This is a very strong and thriving non for profit providing exceptional care in within the NY metro area. We pride ourselves on our top notch patient care and dedication to this specific need in our communities.
Bilingual Chinese Required. Exceptional benefit package, a few of those include, 15k sign on, retirement match, over 7 weeks of PTO, tuition reimbursement. The RN will be mainly responsible for overall case management of specific case load in the Brooklyn area.
(Other boroughs available). Conduct thorough patient assessments including vital signs, physiology evaluation, and medical history review to determine appropriate care strategies. Manage complex case loads involving home based patients in the community.
Collaborate with multidisciplinary teams to develop and implement individualized care plans that meet medical and psychosocial needs. Perform diagnostic evaluations and interpret medical records to inform clinical decisions and treatment adjustments. Engage in patient service activities such as intake assessments, patient education on medication management, and health promotion strategies.
This also includes working closely with the family members. Document all interactions meticulously within electronic medical records using medical terminology for clarity and compliance. Ability to navigate hospital information systems effectively while maintaining compliance with HIPAA standards for patient confidentiality.
Excellent communication skills for coordinating multidisciplinary teams and engaging patients in their own care journey.
