On CALL RN, Hospice Reimbursed

Responsible as a member of the interdisciplinary team to the Nurse Manager and the attending physician for the provision of skilled nursing assessment, planning and care in order to manage the comfort and health of patients and families consistent with Hospice of South Georgia Policies and Procedure. Responsible for providing 24hr skilled nursing intervention and supportive care in patient's home for weekend, holiday, evening and night on-call in a rotating 7on/7off schedule. Utilizing physicians' orders, critical thinking and professional nursing skills, develops and implements a hospice plan of care that meets each patient/family's specific needs.

Formulates this plan in accordance with state and federal guidelines and Hospice of South Georgia Policies and Procedure. Promotes and coordinates team work with physicians and associates; interfaces effectively with other departments and IDT members.
