LPN (Contract-to-Perm | Part-Time Evenings) Location: NY 14456 Contract Dates: 05/12/2026-08/15/2026 Pay: $1450/week Schedule: FT Days and Evenings scheduled as needed (5x8 per week) + EOW and Holiday Position Summary & Responsibilities: Provide direct and indirect nursing care to adult and geriatric residents under RN supervision Collect and report patient/resident physical and psychosocial data Support development and updates of individualized care plans Implement nursing interventions per care plan and scope of practice Communicate changes in resident condition to RN and interdisciplinary team Assist with restorative care, safety, and quality resident outcomes Participate in resident education, family communication, and care coordination Maintain accurate documentation and compliance with facility policies Support infection control and patient safety standards Requirements: Active NYS LPN License required Minimum 1 year LPN experience in LTC setting required BLS required (AHA or RQI only) Must be able to attend full-day orientation (Tuesday or Thursday start) Experience caring for adult and geriatric populations required Knowledge of growth and development across lifespan required Exposure to bloodborne pathogens expected #Workwolf
