Manhattan Home Care Ltd is a licensed home care agency dedicated to providing quality, compassionate care throughout the community. We are seeking experienced and motivated Registered Nurses to join our growing team as UAS Assessors.

Position Summary

The UAS Assessor RN is responsible for conducting in-home Uniform Assessment System evaluations for patients requiring home care services. The RN will complete comprehensive assessments, document findings accurately, and coordinate with enrollment team to ensure appropriate care planning and service authorization.

Responsibilities

  • Conduct face to face UAS assessments in patient's homes and community settings
  • Evaluate patient's functional, medical, cognitive and social needs
  • Explain MLTC plan features and benefits, including explanation of the member's handbook
  • Make enrollment decisions and develop an initial care plan for perspective MLTC member
  • Maintains frequent communication with the Enrollment Department and Care Management
  • Maintains accurate electronic medical records and assessment documentation

Qualifications

  • Active New York State RN license in good standing
  • Bachelor's Science in Nursing Preferred
  • Valid NYS driver's license
  • Home care field assessment experience necessary (1 Year)
  • UAS-NY experience and certification preferred (willing to train)
  • Proficiency in computer skills
  • Ability to work independently, multitask and manage schedule efficiently
  • Reliable transportation and willingness to travel locally
