Position Summary

Plainfield Health Care Center is seeking dedicated and compassionate Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to join our team as Weekend Option Nurses. This position is responsible for delivering high-quality nursing care to residents in a skilled nursing facility, ensuring compliance with all federal, state, and facility regulations while promoting resident dignity, safety, and quality of life.

The Weekend Option Nurse provides clinical oversight, administers medications and treatments, performs assessments, supervises nursing staff, and collaborates with the interdisciplinary team to achieve positive resident outcomes.

Weekend Option Schedule

Day/Evening

  • Saturday: 7:00 AM – 11:00 PM
  • Sunday: 7:00 AM – 11:00 PM
  • Total: 32 hours worked each weekend

Weekend Option premium pay available in accordance with facility policy.

Essential Duties and Responsibilities

Provide direct nursing care to residents in a skilled nursing and long-term care setting.

Complete comprehensive resident assessments and monitor changes in condition.

Administer medications, treatments, wound care, and other physician-ordered services.

Accurately document nursing care, assessments, and resident outcomes in the electronic medical record.

Supervise and direct Certified Nursing Assistants (CNAs) and other assigned staff.

Communicate changes in resident condition to physicians, responsible parties, and appropriate team members.

Participate in care planning and interdisciplinary team meetings.

Respond appropriately to emergencies and resident needs.

Ensure compliance with infection prevention and control standards.

Maintain compliance with CMS regulations, Indiana State Department of Health requirements, and facility policies and procedures.

Promote resident rights, dignity, independence, and person-centered care.

Foster a positive work environment through teamwork and professional communication.

Qualifications

Registered Nurse (RN)

Current Indiana RN license in good standing.

Graduate of an accredited school of nursing.

Skilled nursing facility or long-term care experience preferred.

Licensed Practical Nurse (LPN)

Current Indiana LPN license in good standing.

Graduate of an approved practical nursing program.

Skilled nursing facility or long-term care experience preferred.

Knowledge, Skills, and Abilities

Strong clinical assessment and critical thinking skills.

Ability to prioritize and manage multiple resident needs.

Knowledge of skilled nursing facility regulations and standards of practice.

Strong leadership, communication, and customer service skills.

Ability to work independently and collaboratively within an interdisciplinary team.

Proficiency with electronic medical records preferred.
