  • Salary: $57,000-$78,000 depending on experience and availability

Job

Responsibilities

Conduct client assessments, develop and appropriately revise care plans and collaborate with health care professionals and family to meet client needs Responsible for field staff engaged in the delivery of home care services and monitoring their performance Responsible for the maintenance of complete and accurate client records

Qualifications

Current New York State Registered Nurse (RN) License Minimum of 2 years’ experience in a homecare agency, with at least 1 year in a supervisory position Excellent communication, interpersonal, and problem-solving skills Valid Driver’s license and reliable transportation
