Registered Nurse Navigator - Neurosurgery

Penn State Health - Hershey Medical Center is seeking a Registered Nurse Navigator to help facilitate the patient journey to ensure proper navigation throughout the health care system upon discharge disposition. The RN Navigator supports staff to assist in the coordination of care while the patient is transitioning through the healthcare continuum throughout the outpatient healthcare setting. The RN Navigator serves as the clinical resource to scheduling coordinators and assists in identifying appropriate service or provider for referrals. The RN Navigator will recommend alternative locations of care when appropriate and identify the discharge needs and services.
