**Position

Overview

**

The Patient Care Manager plays a critical leadership role in assisting in overseeing all nursing operations within our hospice program. The PCM is responsible for ensuring the delivery of high-quality patient care, staff management, compliance with regulatory standards, and fostering a culture of compassion and excellence.

Responsibilities

**

  • Help Lead and manage the nursing team, including hiring, training, mentoring, and performance evaluations.
  • Develop and implement policies and procedures to improve patient care and nursing practices.
  • Ensure compliance with federal, state, and local regulations and standards of practice.
  • Collaborate with interdisciplinary teams to create individualized care plans for patients.
  • Monitor patient care outcomes and implement quality improvement initiatives.
  • Provide education and support to nursing staff and other team members regarding hospice care principles and practices.
  • Maintain effective communication with patients, families, and healthcare providers.
  • Participate in community outreach and education to promote hospice services.

  • Bachelor’s degree in Nursing (BSN) or RN license required;

Licensure

**

  • Current Registered Nurse (RN) license in the state of Texas.

Experience

**

  • Minimum of 5 years of nursing experience, with at least 2 years in a leadership or management role in hospice or home health care.
  • **

Skills

**

  • Strong leadership, organizational, and interpersonal skills.
  • Excellent communication and problem-solving abilities.
  • Knowledge of hospice care standards, regulations, and best practices.
  • Ability to work collaboratively in a team-oriented environment.

Job Type: Full-time Pay: $90,000.00 - $100,000.00 per year
