*Job Summary*
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide high-quality home care services to Adult Foster Care and Group Adult Foster Care (AFC/GAFC) patients across diverse settings. The ideal candidate will possess a strong clinical background, excellent patient assessment skills, and a commitment to delivering personalized care in the comfort of patients' homes. As an integral part of our healthcare team, the LPN will ensure safe, effective, and respectful care tailored to individual needs while adhering to all regulatory standards.

- Perform comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation skill.
- Monitor patients' conditions closely, recognizing signs of acute pain or deterioration, and escalate concerns appropriately through agency procedures.
- Assist with activities of daily living such as mobility support, and basic life support during emergencies.
- Collaborate with physicians, case managers, and caregivers to develop and implement individualized care plans aligned with long-term care regulations.
- Maintain meticulous medical documentation in compliance with HIPAA and clinical confidentiality policies.
- Educate patients and their families on health management strategies, including dementia care, Alzheimer's support, and medication administration.
*Qualifications*
- Valid licensure as a Licensed Practical Nurse (LPN) in the relevant state or jurisdiction.
- Prior experience in home care nursing or related settings assisted living environments. Prior Experience with AFC/GAFC Clients is a plus.
- Demonstrated knowledge of infection control protocols and universal precaution procedures.
- Experience working with diverse patient populations including geriatric patients, individuals with developmental disabilities, or those requiring hospice or palliative care.
- Strong communication skills for effective caregiver communication and patient education.
- Ability to handle emergency situations confidently with first aid certification and basic life support (BLS).

Pay: $38.34 - $46.17 per hour Expected hours: 24.0 – 32.0 per week
