The Med Hub Nurse serves as the central clinical liaison between the patient, Emergency Department providers, admitting providers, the Transfer Center, and regional hospitals. This role is responsible for coordinating the admission and transfer process for appropriate low-acuity patients, ensuring efficient communication and seamless transitions of care across the health system. This position requires a caregiver who is empathetic, understanding and nurturing to ensure quality patient care and effective life-saving treatment.
