DaVita is seeking an organized, self-directed RN to manage Home Hemodialysis (HHD) patients. You’ll educate patients and caregivers in-clinic, enabling them to perform dialysis independently at home. This role blends clinical care, patient education, and case management. This hire will also cross-train in Peritoneal Dialysis (PD).

Flexible Monday-Friday 8am-5pm type-of schedule. No Weekends, No Holidays!
Will be covering census in one facility; but must be flexible to support our Westchester area Dialysis Centers as patient needs arise.
Key
Responsibilities
- Train patients and caregivers on HHD procedures and self-care
- Manage a caseload of HHD patients, including assessments and care planning
- Monitor treatment progress, conduct home visits, and provide on-call support
- Collaborate with interdisciplinary teams to ensure safe, high-quality outcomes
Schedule
Monday-Friday, 8-hour shifts
Weekends and holidays off
Flexible scheduling to manage your own caseload
Requirements
RN license in the state of practice
Minimum of 12 months of RN experience
Valid driver’s license, insurance, and reliable transportation
ADN required; BSN preferred
Basic computer skills (MS Word, Outlook)
Preferred

Patient education or teaching background
Case management or telehealth experience
CNN or CDN certification

The Wage Range for the role is $44.00 - $60.00/hour.
Registered Nurse HHD Intermediate: $44.00 - $46.25/hour.
If a candidate is hired, they will be paid at least the minimum wage according to their geographical jurisdiction and the exemption status for the position.
New York Exempt: New York City and Long Island: $66,300.00/year, Nassau, Suffolk, and Westchester counties: $66,300.00/year, Remainder of New York state: $62,353.20/year New York Non-exempt: New York City and Long Island: $17.00/hour, Nassau, Suffolk, and Westchester counties: $17.00/hour, Remainder of New York state: $16.00/hour
