About this role
The Registered Nurse (RN) plays a critical role in supporting the health, well-being, and quality of life of residents across Assisted Living and Memory Care. This position is responsible for clinical assessments, care coordination, documentation, oversight of resident care, and nursing support while ensuring services are delivered with compassion, dignity, and respect for each resident's individuality.

Essential duties and
Responsibilities
Clinical LeadershipWhole-Person Care
- Conduct new admissions, comprehensive assessment, and resident assessments, and ongoing evaluations to support individualized, person-centered care.
- Assist with developing, updating, and implementing resident service plans.
- Ensure residents receive safe, ethical, and high-quality care that reflects their preferences, needs, and goals.
- Obtain, administer, and document medications and treatments in accordance with physician orders, nursing standards, and community policies.
- Monitor resident conditions and recognize changes that may impact health, safety, or well-being.
- Integrate physical, emotional, cognitive, and social well-being into care planning and delivery.
- Support Memory Care best practices that promote dignity, engagement, safety, and meaningful living.
- Ensure residents' needs are met while providing clinical guidance and support to direct care teams.
- Staff training for UMA and UDA employees
Resident ExperienceFamily Partnership
- Foster meaningful relationships with residents and families through compassionate communication and active listening.
- Support residents' independence, choices, routines, and personal preferences when possible.
- Communicate changes in resident condition or care needs to nursing leadership.
- Participate in care discussionscollaborate with nursing leaders regarding care decisions.
- Recognize families as important partners in supporting residents' quality of lifewell-being.
- Reinforce that excellent clinical care and exceptional resident experiences go hand-in-hand.
Medication ManagementClinical Oversight
- Monitor EHR system to ensure accuracy, compliance, and resident safety.
- Review and process physician orders and communicate changes to the Director of Nursing.
- Perform inventory management related to medication administrationpharmacy deliveries.
- Verify and review newly received medications from the pharmacy.
- Support Medication Aides in safe medication administration practices.
- Monitor for medication effectiveness, adverse reactions, and potential concerns.
DocumentationCompliance
- Maintain resident records, files, assessments, and documentation accurately and timely.
- Prepare necessary medical paperwork for appointments and care coordination.
- Ensure incident reports are completed accurately, promptly, and according to policy.
- Assist with regulatory compliance and survey readiness efforts.
- Support state licensure surveys and compliance reviews.
- Ensure documentation reflects care provided and meets regulatory standards.
Team LeadershipCulture Building
- Promote and support a positive work environment focused on teamwork, collaboration, and shared accountability.
- Serve as a clinical resource for team members during shifts.
- Assist with onboarding, training, and competency development when requested.
- Communicate important resident updates through established communication systems.
- Foster a culture of compassion, professionalism, and continuous improvement.
- Assist with staffing support and shift coverage as needed to ensure continuity of care.
Quality AssuranceRisk Management
- Observe and document significant conditions, changes, and responses to treatments.
- Take and monitor vital signs.
- Identify potential risks and collaborate on interventions that improve resident outcomes.
- Support quality improvement initiatives and best practices in resident care.
- Practice sound clinical judgment and ethical decision-making.
OperationalCommunity Support
- Provide on-call support and clinical consultation as assigned.
- Partner with the leadership team to support community goals and quality outcomes.
- Assist with maintaining effective communication among all departments.
- Participate in meetings, education opportunities, and professional development activities.
- Support a culture where residents and team members feel respected, informed, and valued.
General
Requirements
- Active and unencumbered Registered Nurse (RN)
- Minimum of two (2) years of experience in a healthcare setting.
- Strong interpersonal, verbal, and written communication skills.
- Knowledge of compliance, regulatory requirements, and clinical best practices.
- Strong critical thinking and problem-solving abilities.
- Must submit to a TB test in accordance with state regulations.
- Ability to participate in on-call responsibilities as assigned.
- Ability to manage multiple priorities and adapt to changing resident needs.
- Must maintain confidentiality of resident, family, and organizational information.
- Must exercise sound judgment, professionalism, and ethical conduct at all times.
