Available Shifts

We are currently hiring for the following shifts: 8 to

• 10 hour shifts

Week 1

• Thursday: 1100-2100

• Friday: 0900-1700

• Saturday: 0900-1700

Week 2

• Thursday: 1100-2100

• Friday: 0900-1700

Part-time team members are required to work a minimum of one shift per week.

Position Summary

As a Center One Community Team nurse, you will provide skilled nursing services that promote health, safety, independence, and quality of life. You will work collaboratively with caregivers and the interdisciplinary team to deliver individualized care based on each person's needs and Individual Support Plan (ISP).

Essential Responsibilities

• Provide skilled nursing care in accordance with physician orders and Individual Support Plans (ISP)

• Administer medications and treatments while monitoring for effectiveness and adverse reactions

• Perform comprehensive nursing assessments and monitor changes in health status

• Monitor and document vital signs, neurological status, and overall condition

• Assist individuals with activities of daily living (ADLs) while promoting independence and dignity

• Maintain accurate, timely, and compliant clinical documentation

• Educate individuals, families, and caregivers regarding health needs and care plans

• Collaborate with physicians, supervisors, direct support professionals, and interdisciplinary team members

• Respond appropriately to emergencies and changes in medical condition

• Follow all infection prevention, HIPAA, and Center One Community Team policies and procedures

QualificationsRequired

• Current Pennsylvania Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in good standing

• Current CPR/BLS and ACLS Certification

• Valid driver's license, reliable transportation, and current automobile insurance

• Strong communication, organizational, and documentation skills

• Ability to work independently while collaborating effectively with the healthcare team

Preferred

• Experience working with individuals with Intellectual and Developmental Disabilities (IDD)

• Home health or private duty nursing experience

• Experience with tracheostomy and ventilator care

• Experience caring for individuals with complex medical needs

• New graduates are encouraged to apply

Clinical Skills Preferred

Experience with any of the following is a plus

• Medication administration

• Seizure precautions

• Complex neurological
