About this role
The Integrated Services Navigator is responsible for providing culturally competent navigation services to patients needing behavioral health or nutritional counseling appointments. The Navigator is expected to build working relationships, provide, and connect patients with an integrated services provider, while practicing excellent customer service. This position solves problems and supports patients throughout the various points from appointment scheduling to completion of visit.

Essential Functions

- Conducts face-to-face screenings to schedule patients eligible for behavioral health or nutritional services.
- Identifies, evaluates, and resolves real and perceived barriers to care, scheduling and completion of integrated services appointments.
- Utilizes evidence-based motivational interviewing tools to encourage patients to schedule and complete integrated services visits.
- Maintains accurate and complete documentation in the patient records.
- Evaluates patient concerns, complaints, and trends to recommend changes to accommodate patient needs and eliminate barriers.
- Maintain regular and predictable attendance.
- Performs other duties as required.

Qualifications

- High School Education or GED (General Education Degree) equivalent.
- One (1) year or more experience in Electronic Medical Records.
- Valid AZ driver’s license.

- One (1) year or more of Medical Assistant back-office experience or patient navigator.
