We’re looking for a compassionate, dependable Hospice LPN to join our team and provide exceptional, person-centered care in the community.

What You’ll Do

Provide skilled nursing care to hospice patients in their homes and other care settings.

Monitor patient conditions and communicate changes to the hospice care team.

Administer medications and treatments as directed by the plan of care.

Educate and support patients and families with compassion, patience, and respect.

Work collaboratively with RNs, providers, social workers, chaplains, aides, and other members of the interdisciplinary team.

Help patients remain comfortable and supported while honoring their goals and preferences.

Complete accurate clinical documentation and follow hospice standards of care.

What We’re Looking For

Current LPN license in Maine.

Strong clinical skills and sound judgment.

Excellent communication and organizational skills.

A compassionate, respectful approach to patients and families.

Ability to work independently while being an engaged member of a collaborative team.

Hospice, home health, or palliative care experience is valued, but we’re also interested in LPNs who are passionate about learning hospice care.
