Responsibilities

  • Conduct comprehensive patient assessments including vital signs, physical examinations, and medical history reviews to determine care needs.

  • Administer medications and injections accurately following established protocols, including IV infusions and medication management.
  • Perform specialized procedures such as suturing, catheterization, tube feeding, and sterile dressing changes using aseptic techniques.
  • Collaborate with multidisciplinary teams to develop and implement individualized care plans aligned with patient goals and medical directives.
  • Utilize electronic health record (EHR) systems such as Epic, Cerner, Athenahealth or eClinicalWorks to document patient information meticulously and ensure compliance with HIPAA regulations.
  • Monitor patients in various settings including ICU, telemetry units, emergency rooms, long-term care facilities, hospice environments, and outpatient clinics.
  • Provide patient education on health coaching topics such as medication administration, chronic disease management (e.g., diabetes or cardiac conditions), and post-discharge instructions.
  • Support triage activities by prioritizing urgent cases in emergency or urgent care settings while maintaining high standards of infection control and safety protocols.

Skills

  • Proven experience working with diverse populations including pediatrics, geriatrics, individuals with disabilities or developmental delays, and memory care patients.
  • Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), CPT coding, and laboratory procedures.
  • Proficiency in EMR/EHR systems such as Cerner or Epic for accurate documentation and medical records management.
  • Skilled in critical care practices including trauma medicine (Level I & II trauma centers), ICU experience, neonatal resuscitation procedures (NRP), ventilator management, and acute pain management strategies.
  • Competence in performing diagnostic evaluations like sonography or laboratory specimen collection/processing when required.
  • Ability to handle complex cases involving infectious disease care, hospice & palliative medicine, occupational health issues including Workers’ Compensation law compliance.
  • Excellent patient assessment skills combined with effective communication for delivering high-quality nursing care in hospital medicine or outpatient settings.
  • Experience working with EMR/EHR systems such as Athenahealth or eClinicalWorks; familiarity with HIPAA standards for safeguarding patient information is essential.
