Responsibilities: Conduct initial and comprehensive resident assessments using the Minimum Data Set (MDS) and develop individualized care plans Lead and participate in care plan conferences to ensure comprehensive care Collaborate with the interdisciplinary team to develop and implement resident care plans Identify and prioritize resident needs, and coordinate care to meet those needs
