*Job Summary* We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our 5-Star healthcare team. The ideal candidate will provide essential nursing care to patients in various settings, including hospitals and home care environments. The LPN will work under the supervision of registered nurses and physicians, ensuring that patients receive high-quality care tailored to their individual needs.

  • Conduct physical examinations and assess patient health status.
  • Administer medications and perform treatments as prescribed by physicians.
  • Assist with family planning and educate patients on health management.
  • Provide home care services, including IVs, Wound care, and education.
  • Perform phlebotomy for blood draws and laboratory testing.
  • Monitor vital signs and report any changes in patient conditions.
  • Collaborate with healthcare teams to develop and implement patient care plans.
  • Maintain accurate patient records and documentation in compliance with regulations.
  • Uphold safety standards and infection control protocols within the healthcare environment.

*Requirements*

  • Valid Licensed Practical Nurse (LPN) license in the state of practice.
  • Valid CPR, Drivers License, IV Certificate and Auto Insurance.
  • Knowledge of physiology and hospital medicine is preferred.
  • Experience in home care settings is a plus.
  • Strong interpersonal skills with the ability to communicate effectively with patients and families.
  • Ability to work collaboratively within a healthcare team.
  • Compassionate nature with a commitment to providing quality patient care.
  • Willingness to learn new skills and adapt to changing healthcare environments.
