Are you an experienced Registered Nurse (RN) looking for a rewarding and flexible temporary role? We are seeking a compassionate and detail-oriented Residential Nurse Case Manager to join our team for a 2–3-month assignment starting in June 2024. This is a unique opportunity to provide crucial oversight and hands on support as we begin to prepare for our Department of Developmental Services period, making a meaningful impact on the lives of the individuals we support.

Responsibilities

Training

Conduct necessary training sessions for residential staff on all health-related protocols to ensure proper care and quality standards and best practice.

Client Health Oversight

Ensure all medical protocols are updated, signed, and uploaded to our electronic health record system. Liaison between the Health Care Providers, individuals, and program staff. Conduct weekly physical assessments for assigned individuals and provide updates on each visit.

Medication Oversight

Ensure medication orders are on the correct form and signed by the prescriber. Verify all medication orders are accurately reflected on the Medication Administration Record (MAR). Confirm that all PRN medications have clear parameters and that medical protocols are current and followed. Check the Control Count Book for accuracy and proper completion. Identify and report any medication errors to the administrative team promptly.

Audit and Compliance

Audit health and medication records to ensure all initial and follow-up appointments are completed as recommended by healthcare providers. Maintain compliance with all health protocols and documentation standards.

Qualifications

Valid RN license in Massachusetts. Experience in residential or case management nursing preferred. Strong attention to detail and organizational skills. Proficiency with electronic health record (EHR) systems. Familiarity with the Medication Administration Program (MAP) is a strong advantage. Excellent communication and training skills. Ability to work independently and manage time effectively.
