The Field Manager is responsible for planning, directing, and supervising the delivery of high-quality patient care within our Home Health team. This role works closely with clinical leadership to support nursing staff, ensure compliance with care standards, and coordinate services to meet patient needs.

Responsibilities
Supervise and support Home Health Nurses to ensure quality patient care
Coordinate case management and interdisciplinary services for patients and families
Conduct field visits with clinicians to patient homes
Monitor quality, compliance, and appropriateness of care delivery
Provide back-up support to the Clinical Manager as needed
Assist with onboarding, orientation, and ongoing staff education
Participate in QAPI initiatives and in-service programs
Ensure adherence to infection control, safety, and organizational policies
Qualifications
Active RN license required
Bachelor’s degree in Nursing (BSN) required or in progress
Minimum 2 years of community health or home care experience
Strong computer and documentation skills
Excellent organizational, communication, and leadership abilities
Working Conditions
Combination of office and field-based work
Regular travel to patient homes
In compliance with New Jersey's Pay Transparency Act, the compensation range for this position is USD $84,000 to USD $112,000.
