Essential Job Functions

  • Maintains and actively promotes effective communication with all individuals.
  • Maintains a positive image of the entity in the community keeping in alignment with our mission, vision, and values.
  • Responsible for on-call services, as assigned.
  • Manage the staffing plan through recruitment, selection and performance management to ensure the effective delivery of programming and services.
  • Supervise and direct the care of other nursing personnel including registered nurses, licensed practical nurses and certified nurse aides.
  • Support and triage patient care visits and clinical questions and serve as the registered nurse case manager as needed.
  • Leads the Quality Assurance process along with Oasis programming and guidance to nursing staff to meet regulatory standards and outcomes.
  • Performs and coordinates the Clinical Record Review with all disciplines.
  • Assist with utilization reviews to ensure the development, implementation, and updates of the individualized plan of care.
  • Assist Director in monitoring and maintaining acceptable clinical outcomes including documented quality improvement initiatives and outcome data.
  • Responsible for compliance with all local, state and federal codes pertaining to the operations of the agency.
  • Follow safety policy and procedures, monitoring client and employee incidents/accidents with appropriate follow up actions.
  • Practice infection control mitigation at all times.
  • Maintain honest, open, and professional communication with others including promoting the essence of the interdisciplinary team.
  • Respond by phone to patient and family calls in a timely manner.
  • Perform other duties as assigned.

Requirements

Experience & Training

  • Must be currently licensed as a Registered Nurse in the State of Wisconsin.
  • Working knowledge of Federal and State Home Health regulations.
  • Comprehensive knowledge of OASIS, Medicare process and regulations.
  • 2 – 3 years of experience in Home Health preferred.
  • Prior quality assurance experience preferred.

Location

  • On-Site (Commonheart Office - N16W23217 Stone Ridge Drive Suite 350 Waukesha, WI 53188)

Schedule

  • Monday to Friday: 8:00 AM to 4:30 PM

Pay

  • $92,809.60–$110,614.40 per year (Higher compensation may be considered based on qualifications and experience)
