*Job Summary*
Visiting Nurse and Hospice for Vermont & New Hampshire is seeking an experienced *Travel RN Case Manager – Home Health* to provide compassionate, patient-centered care in the home setting. The RN Case Manager will assess, plan, coordinate, and manage individualized patient care while collaborating with physicians, caregivers, and an interdisciplinary healthcare team to ensure the best possible outcomes.
*Requirements*
- Active *Vermont and New Hampshire RN License* or an *unencumbered Compact (Multistate) RN License*.
- Graduate of an accredited School of Nursing.
- Minimum *2 years of recent Home Health RN experience*.
- Current *BLS* certification.
- Strong assessment, case management, communication, and documentation skills.
- Experience with *EPIC EMR* preferred.
