We are seeking a Licensed Practical Nurse (LPN) or Registere Nurses (RN) with a strong back-ground in public health, home health care, or hospital settings. This role is essential in delivering one-on-one, patient-focused care while maintaining the highest standards of nursing practice. Job Description: Focused Nursing for Our Patients Charity Care Group seeks a committed and skilled Licensed Practical Nurse (LPN) and Registered Nurse (RN) to deliver skilled nursing services in a day services environment or home enviroment, ensuring adherence to the highest standards of care. This role demands a balance of clinical expertise and compassionate patient interaction, complying with agency standards and regulations. Responsibilities Licensed Practical Nurse (LPN) or Registered Nurse (RN) needed for skilled nursing support for a Intellectual and Developmentally Disabled Persons attending Day Support programs. Monitoring needed during Day Support hours. Manage G‑Tube care, including medication administration and feedings per care plan. Monitor, document, and report any changes in condition to the supervising nurse/agency. Collaborate with Day Support staff and family to ensure safety and quality of care. As a LPN or RN, you will oversee the delivery of skilled nursing care and coordinate with healthcare teams to ensure adherence to our rigorous standards. You will play a key role in implementing and supervising patient care plans while driving quality improvement initiatives. Schedule is Monday–Friday, 9:00 a.m.–4:00 p.m. Status: PRN, approximately 3–5 days per week. Weekends PRN • Skilled Nursing Care: Provide services such as medication management, wound care, and respiratory support in day services environment. • Patient Education: Educate patients and their families on care plans and health management. • Documentation and Compliance: Maintain accurate records and adhere to all regulatory standards. • Collaborative Care: Work closely with RNs, medical staff, and families to ensure seamless care delivery. Candidate Profile: Dedicated and Compassionate Professional Qualifications: LPN or RN license in the state of Virginia, current CPR certification, experience with G‑Tube care preferred. Previous experience with adults with developmental or intellectual disabilities is a plus! Compensation $26-46 per hour. **** Current LPN or RN license in Virginia, with graduation from an accredited nursing program****** Experience: At least 2 years of experience in home health care, skilled care, or a related field. • Transportation: Reliable transportation, a valid Virginia driver’s license, and car insurance. • Clinical Skills: Familiarity with home health equipment and patient care documentation. • Empathy and Adaptability: A compassionate approach to patient care and the ability to adapt to individual needs Please send your resume and availability #Workwolf
