Hospice RN Case Manager – $85K–$91K | Mileage + Benefits

What You’ll Do

*

• Provide skilled nursing care and case management for hospice patients
• Develop and implement individualized plans of care
• Coordinate with the interdisciplinary team to ensure continuity of care
• Educate and support patients and families through the hospice journey
• Maintain accurate, timely documentation aligned with regulatory standards
• Respond to changes in patient condition with sound clinical judgment

*The Nurse We’re Seeking:*

• ✅ Active Registered Nurse (RN) license in the State of Texas
• ✅ Minimum three (3) years of hospice experience required
• ✅ Strong understanding of hospice regulations and documentation standards
• ✅ Confident in patient and family education
• ✅ Ability to manage a caseload with organization and consistency
• ✅ Sound clinical judgment and decision-making skills
• ✅ Professional presence in homes, hospitals, and facilities
• ✅ High standards for documentation accuracy and quality care
