About the Role:
Panacea Home Healthcare LLC is looking for a ONE-ON-ONE dedicated and compassionate RN or LPN to join our growing team in White Oak, OH. This is a rewarding opportunity to make a real difference in patients' lives by delivering high-quality, personalized care right in the comfort of their homes. Responsibilities:
Provide direct patient care in home settings, including assessments, treatments, and medication administration
Develop, implement, and monitor individualized patient care plans
Monitor and document patient health status, progress, and changes in condition
Communicate effectively with physicians, care teams, and family members regarding patient needs
Educate patients and caregivers on disease management, medications, and home safety
Maintain accurate and timely clinical documentation in compliance with agency and regulatory standards
Requirements:
Active RN or LPN license in the state of Ohio (in good standing)
Prior home health, long-term care, or clinical nursing experience preferred
Strong clinical assessment and critical thinking skills
Excellent communication and patient education abilities
Reliable transportation and valid driver's license for home visits
Ability to work independently and manage the patient
CPR/BLS certification required or ability to obtain upon hire
