Responsibilities
Participates in the planning, development and implementation of harm reduction. Build relationships with internal departments as well as outside agencies. Provides appropriate case management as needed.
Provides referrals to housing, treatment for substance use disorder. Provide education on a variety of topics that encompass harm reduction. Incorporate other aspects of Harm reduction to include syringe, safe use supply and exchange, HIV/HCV testing, overdose prevention /response, safer sex education and supplies, wound care education and supplies.
Develop long and short-term relationships with those we serve with the goal of harm reduction. Devise a service plan with the clients and include short and long term, reasonable and attainable goals for individuals that we serve. Appropriate work attire is required.
Must attend all mandatory department trainings. Ensures confidentiality of financial records and all records for employees and/or clients. Displays a responsive and professional manner in promptly responding to all requests, complaints, and problems.
Recognizes that each employee is a representative of the Fond du Lac Band of Lake Superior Chippewa and is responsible for demonstrating courtesy, respect, and sensitivity to the needs of all others, including visitors and co-workers. Represents the Fond du Lac Band of Lake Superior Chippewa and the department in a positive and professional manner in the community. Required to maintain proper attendance including reporting to work on time in accordance with applicable policies.
Maintains a clean and organized work area.
- Due to changes and modifications in the job from time to time, employees are required to be flexible and assume other responsibilities assigned by management as deemed necessary.
Qualifications
Valid Driver’s License issued in the US is required.
Eligibility determined by FDL's insurance company.
Associate Degree in Nursing is required, BA/BS in Nursing preferred; An Unrestricted Minnesota Registered Nurse License is required and kept in good standing for duration of employment.
2 years’ experience in nursing is required.
Wound care certification is required or must be obtained within 3 months of employment.
Current Basic Cardiac life support (BCLS) is required.
A deep understanding of and respect for harm reduction model and its principles is required.
Certification in Nurse Case Management is preferred.
Certification in public health is preferred.
Must be available to do nights and weekends when requested.
Experience with working with people living with HIV and people who use drugs is preferred.
Knowledge of computer applications to include word processing, databases and/or data entry, Spreadsheets, and Microsoft Outlook is required.
Knowledge of Native American communities, culture, customs, and beliefs is preferred.
Experience working with Native Americans clients, communities and is able to demonstrate cultural sensitivity is preferred.
Ability to communicate effectively orally and in writing is required.
Ability to work independently and establish priorities is required.
Attention to detail and accuracy is required.
Ability to establish professional and harmonious working relationships on all projects and with all parties involved.
Subject to drug and alcohol testing in accordance with the Fond du Lac Reservation’s Personnel Policies.
Subject to pre-employment and annual background checks.
Subject to immunization review and submitted proof of required vaccinations.
Travel is required.
