About this role

Premier Rehab & Healthcare at Burlington is seeking an experienced and dedicated MDS Coordinator to join our team. The MDS Coordinator plays an essential role in ensuring accurate resident assessments, comprehensive care planning, regulatory compliance, and high-quality, resident-centered care.

Responsibilities

Coordinate the development and completion of comprehensive resident assessments

Read the full posting

Complete MDS assessments in accordance with current state and federal regulations

Conduct and coordinate Care Area Assessments (CAAs)

Coordinate and support individualized resident care plans

Collaborate with nursing, therapy, social services, dietary, physicians, and other members of the interdisciplinary team

Ensure MDS documentation is accurate, complete, and submitted within required timelines

Monitor MDS schedules and regulatory deadlines

Participate in facility survey preparation and the survey process

Review resident documentation for accuracy and completeness

Communicate effectively with residents, families, and interdisciplinary team members

Support quality improvement and regulatory compliance initiatives

Qualifications

Current, unencumbered Vermont RN or LPN license

Previous MDS and PPS experience required

Previous care planning experience preferred

Strong knowledge of MDS, PPS, and applicable regulatory requirements

Excellent assessment, documentation, organizational, and communication skills

Strong attention to detail and ability to meet deadlines

Ability to work effectively as part of an interdisciplinary team

Commitment to providing high-quality, resident-centered care
