Compassionate Per Diem Nurse Needed for Home Health Care
Are you a dedicated and experienced Registered Nurse (RN) or Licensed Practical Nurse (LPN) seeking flexible per diem opportunities? Home-Based Care: Provide personalized care to clients in the comfort of their own homes.

Duties
Depending on the client's needs, duties may include Medication administration, Wound care, Vital sign monitoring, Patient education, Assistance with activities of daily living (ADLs), and Monitoring and reporting patient conditions.
Collaborative Environment: Work as part of a team to ensure optimal patient outcomes. Serving: Phoenix/Peoria/Suncity/Glendale
In this role, you will primarily create and manage a care plan for individual patients. You are a Registered Nurse (RN) who operates with great attention to detail and a big heart. You take pride in creating personalized care plans for patients and can observe and communicate effectively in your patients' best interests. To succeed, you should have demonstrated knowledge and skill in current nursing practices as well as a strong penchant for solving problems.
Responsibilities
- Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans
- Administer medications and monitor patients for side effects or reactions
- Document any changes in patient conditions and report on changes
- Maintain accurate records of patient care and discuss observations with the supervisor
Qualifications
- Currently licensed Registered Nurse (RN) in state of employment
- Previous experience as a Registered Nurse is preferred
- Excellent interpersonal and client care skills
- Valid driver’s license
- First aid/CPR certification required
