Evening Shift LPN Job Summary: We are seeking a compassionate and skilled LPN to join our evening shift team at Bradford Manor Nursing and Rehabilitation. As an LPN on our team, you will play a vital role in providing high-quality skilled nursing and rehabilitation services to our residents. If you are dedicated to delivering exceptional patient care and are looking for a rewarding career, we encourage you to apply for this exciting opportunity. Responsibilities: • Provide direct patient care, including administering medications, performing wound care, and assisting with activities of daily living • Collaborate with the interdisciplinary team to develop and implement individualized care plans • Monitor and report patient conditions, including vital signs, to the charge nurse and physicians • Assist with tube feedings, IV therapy, and other treatments as needed • Communicate effectively with patients, families, and healthcare team members to ensure seamless care transitions • Participate in quality improvement initiatives to continually improve patient outcomes and satisfaction Requirements: Current LPN licensure in the state of Pennsylvania High school diploma or equivalent required 1+ years of experience in long-term care or rehabilitation setting (n/a) Working Conditions: This position requires flexibility to work the evening shift (n/a).
