We are seeking a compassionate and skilled LPN to join our evening shift team at Bradford Manor Nursing and Rehabilitation. As an LPN on our team, you will play a vital role in providing high-quality skilled nursing and rehabilitation services to our residents. Responsibilities
- Provide direct patient care, including administering medications, performing wound care, and assisting with activities of daily living
- Collaborate with the interdisciplinary team to develop and implement individualized care plans
- Monitor and report patient conditions, including vital signs, to the charge nurse and physicians
- Assist with tube feedings, IV therapy, and other treatments as needed
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care transitions
- Participate in quality improvement initiatives to continually improve patient outcomes and satisfaction
Requirements
Current LPN licensure in the state of Pennsylvania
High school diploma or equivalent required
1+ years of experience in long-term care or rehabilitation setting (n/a)
Working Conditions
