Hospice LPN

Trustbridge Hospice, a member of Empath Health, is seeking a compassionate and dedicated Hospice Licensed Practical Nurse (LPN) to join our team and provide high-quality, patient-centered care to individuals receiving hospice care in the Palm Beach County area. You will work 8:30am - 5pm with weekend availability. Since 1978, Trustbridge Hospice has been honored to serve families across Palm Beach and Broward counties with compassionate, community-based care.

Formed from the legacy of Hospice by the Sea and Hospice of Palm Beach County, we continue to provide comfort, dignity, and peace to those facing advanced illness. What you'll Do As a Hospice LPN, you will play a vital role in supporting patients and families during some of the most critical moments in care. Work under the direction of the Registered Nurse and primary physician to assess patient and family needs Coordinate and provide individualized care plans that prioritize comfort and dignity Utilize monitoring, intervention, and teaching skills to enhance quality of life Provide emotional support and reassurance to patients and families during overnight care Ensure exceptional, personalized service and a positive experience for every patient and family Why Join Empath Health?

What You'll Need

Current license to practice as a Licensed Practical Nurse in the State of Florida. Current Basic Life Support Certification. Successful completion of 30 hour IV Course meeting State of Florida Board of Nursing requirements is a plus.
