Admissions and visits for home health

Duties

  • Provide direct patient care, including assessments, evaluations, and treatments

  • Administer medications and treatments as prescribed by physicians
  • Monitor patient vital signs and report any changes or abnormalities
  • Assist with procedures such as wound care, wound VAC, Intravenous medications, medications administration.
  • Collaborate with other healthcare professionals to develop and implement patient care plans
  • Educate patients and their families about health conditions, medications, and self-care techniques
  • Document patient information accurately and maintain medical records
  • Ensure compliance with all healthcare regulations and protocols

Qualifications

  • Valid Registered Nurse (RN) license in the state of [location]
  • Bachelor's degree in Nursing (BSN) preferred
  • Experience working with diverse patient populations, including family planning, toddler care, special needs, etc.
  • Proficiency in using electronic medical record systems
