About this role
Aurora Care Hospice is seeking a Hospice Nurse (RN Case Manager) to provide compassionate, comfort-focused care for patients with life-limiting illness. The role involves serving as RN Case Manager for an assigned hospice caseload, managing the plan of care from admission through end of life, performing skilled nursing visits, symptom and pain management, medication management, and hands-on comfort care. The position starts on a per diem/PRN basis with a path to full-time status and benefits eligibility as patient volume grows.

- RN
- Active unrestricted Michigan RN license
- Minimum of one to two years of clinical nursing experience
- Hospice, home health, or acute/ICU experience preferred
- CHPN certification is a plus but not required
- Valid driver's license
- Reliable transportation

- Proficiency with EMR documentation
