Registered Nurse (RN) Case Manager – Floating Territories position at Home Health Care, Inc. Provides skilled nursing support across multiple home health territories throughout the Twin Cities metro area. Completes OASIS assessments and individualized plans of care, delivers one-on-one skilled nursing visits and patient education, collaborates with physicians and interdisciplinary team members, and supports local teams during staffing needs.

Requirements

Active Minnesota Registered Nurse (RN) license
Strong clinical assessment and critical thinking skills
Comfortable adapting to different territories and patient populations
Home health experience preferred, but not required
Reliable transportation and willingness to travel throughout the metro
OASIS assessments
Clinical documentation
