Wise Choice Home Health is seeking a compassionate, dependable Licensed Vocational Nurse (LVN) to provide skilled nursing care to patients in their homes throughout the Fresno area. This per diem position offers flexible scheduling based on your availability, patient needs, and agency census.
The ideal candidate is organized, clinically strong, and committed to providing safe, respectful, patient-centered care while working closely with the supervising RN and interdisciplinary care team.
Responsibilities
● Provide skilled nursing visits according to physician orders, the established plan of care, agency policies, and LVN scope of practice. ● Collect and document focused clinical data, including vital signs, symptoms, treatment response, and changes in condition. ● Provide ordered nursing care such as wound care, medication administration, injections, diabetic care, catheter care, and patient or caregiver education within demonstrated competency. ● Reinforce education regarding medications, treatment plans, disease management, safety, and infection prevention. ● Promptly report changes in the patient’s condition or concerns to the supervising RN and appropriate provider. ● Complete accurate, timely electronic documentation for every visit. ● Communicate and coordinate effectively with patients, families, the RN case manager, providers, and other members of the care team. ● Follow all infection-control, safety, privacy, and regulatory requirements.
Qualifications
● Active, unrestricted California LVN license. ● Current BLS/CPR certification. ● At least one year of nursing experience preferred; home health experience is strongly preferred. ● Current California driver’s license, reliable transportation, and automobile insurance. ● Ability to travel between patient homes throughout the assigned Fresno-area territory. ● Strong clinical judgment, communication, organization, and time-management skills. ● Ability to work independently while following the established plan of care and clinical supervision. ● Ability to complete all required pre-employment health and background clearances. ● Bilingual skills are a plus but are not required.

Job Summary
Wise Choice Home Health is seeking an experienced, compassionate Registered Nurse (RN) to provide skilled nursing services to patients in their homes throughout the Fresno area. This per diem role offers flexible scheduling based on your availability, patient needs, and agency census.
The ideal candidate is confident working independently in the field, provides excellent patient and caregiver education, and communicates effectively with providers and the interdisciplinary care team.
Responsibilities
● Perform skilled nursing visits, including start-of-care, resumption-of-care, recertification, routine, and discharge visits, as assigned and qualified. ● Complete comprehensive nursing assessments and required OASIS documentation, when applicable. ● Develop, implement, evaluate, and update individualized plans of care in coordination with the ordering provider and interdisciplinary team. ● Provide skilled nursing care, including wound assessment and treatment, medication reconciliation and management, injections, diabetic care, disease-management education, and other ordered services within scope and competency. ● Educate patients and caregivers regarding diagnoses, medications, treatments, safety precautions, infection prevention, and when to seek additional care. ● Monitor patient progress, identify changes in condition, and communicate or escalate concerns promptly. ● Coordinate care with physicians, therapists, LVNs, home health aides, pharmacies, and other involved services. ● Provide clinical supervision and direction to LVNs and home health aides when assigned. ● Complete accurate, timely electronic documentation and maintain compliance with agency, payer, and regulatory requirements. ● Participate in care coordination, case conferences, quality initiatives, and required training.
