Position Overview

JOB TITLE: Case Management Manager REPORTS TO: Chief Administrative Officer

JOB

Summary

Supervises and directs case management staff in provision of care. Works closely with physicians, nurses, and associated clinical professionals to help patients in an acute setting transition to a lower level of care. Helps to remove barriers from the patient's timely discharge from the hospital.

Resolves clinical, psychosocial and financial barriers on the patient's behalf. Handles difficult situations in a way that promotes the best outcomes for the patient while reducing the facility's overall cost-to-care. Has principal responsibility for total nursing care, assessment, planning, implementation and evaluation of each assigned patient.

Plans, provides and documents discharge planning, teaching and educating patients, family or others to attain optimal health and continuity of care. Assures that the plan of care is carried out in a safe environment through performance and supervision of subordinate personnel. OSHA Bloodborne Pathogens Class I.

Essential duties and

Responsibilities

  • Conducts discharge assessment, beginning on admission to evaluate patient's current and future concerns.
  • Facilitates and coordinates post-hospital care.
  • Follow up phone call with each patient within three days post discharge.
  • Interacts and supplements the health care team to enhance the social, emotional and psychological care of patient/family/significant others.
  • Provides timely and appropriate discharge plans for patients.
  • Maintains documentation of discharge services provided.
  • Keeps physician informed regarding discharge plans of patient.
  • Keeps database of referral sources.
  • Refers patient, client, or family to community resources to assist in recovery from physical illness.
  • Provides access to services such as financial assistance, advanced directives, legal aid, or education.

Education

Graduate of an accredited School of Professional Nursing.

B. Personal job-related

skills

Effective oral and written communication skills. Highly organized, empathetic, and capable of multitasking in a high-stress environment.

C. Licensure, registry, or

certification

Registered Nurse currently licensed by the State of Texas. Possess and maintain Basic Life Support Certification through American Heart Association. ACLS preferred.

D. Experience

a. Prior Work

Experience

Management role in the Case Management department preferred. Five years of experience as a professional Registered Nurse preferred.

b. Technical Training: Current CPR training

E. Physical and Mental

Requirements

Duties of the position require extended periods of standing and walking. Bending, stooping, twisting, reaching, and lifting up to 50 pounds. Occasional heavy lifting when moving or supporting patients who may suddenly become weak or helpless.

Pay: $93,500.00 - $103,000.00 per year Work Location: In person
