Absolute Care Health Systems, Inc., D. B. A Shield Home Health, seeking to find dedicated, high-quality, independent Home Health RN Case Manager candidates to perform visits at clients homes.
Case Management responsibilities include independently managing delivery of care to clients by interdisciplinary team (LVNs, Home Health Aides, Social Worker, Chaplain) on each case load. While role does not directly manage other staff the candidate is supervising other professionals to ensure client needs are being met.
Duties Include
- Completing initial and ongoing comprehensive assessments of patient and family to determine needs.
- Provide professional nursing care.
- Initiate the plan of care and make necessary revisions as patient status and needs change.
- Counsel the patient and family in meeting nursing and related needs.

Requirements
- Graduate of an accredited school of nursing
