About the Role
As a Hospice Registered Nurse , you'll be an essential part of our care team, providing compassionate, high-quality care to patients and families in the comfort of their homes, assisted living communities, and skilled nursing facilities. You'll use your clinical expertise, compassion, and understanding of the hospice philosophy to help patients receive the comfort and support they deserve.
We're looking for someone who is dependable, compassionate, solutions-oriented, and committed to delivering excellence. Whether working directly with patients, supporting operations, or partnering across departments, you'll have the opportunity to contribute in meaningful ways while continuing to grow your career.
What You'll Do
Provide Exceptional Patient Care
- Provide high-quality bedside care to patients and their families.
- Deliver compassionate care in patients' homes, assisted living communities, and skilled nursing facilities.
- Manage patient symptoms with clinical expertise and empathy.
- Support patients and families throughout their hospice journey.
Educate & Support Patients and Families
- Provide education on disease progression, end-of-life care, self-care techniques, and nutrition .
- Help patients and families understand their care and what to expect throughout the hospice journey.
- Provide compassionate support while honoring each patient's needs and wishes.
Collaborate with Your Care Team
- Work closely with the interdisciplinary team to ensure comprehensive care .
- Communicate and collaborate with other members of the care team to support patient needs.
- Help ensure patients receive coordinated, compassionate hospice care.
Document & Coordinate Care
- Accurately document assessments, progress notes, care plans, and physician orders.
- Maintain complete and accurate patient records.
- Communicate patient needs and changes in condition appropriately with the care team.
Qualifications
- Experienced Hospice or Home Health RN.
- Strong understanding of the hospice philosophy.
- Excellent communication and observation skills.
- Compassion and dedication to providing exceptional patient care.
- Ability to provide care in patients' homes, assisted living communities, and skilled nursing facilities.
