Job Overview We are seeking a dedicated and energetic Part-Time RN Care Coordinator to join our healthcare team. In this vital role, you will serve as a key liaison between patients, families, and healthcare providers, ensuring seamless coordination of care across various settings. Your expertise will help improve patient outcomes, enhance care experiences, and promote efficient utilization of healthcare resources. This position offers a flexible schedule for registered nurses passionate about making a meaningful difference in patient lives through proactive case management and comprehensive care planning.
Responsibilities.
1.. Be a registered nurse (R. N.) with a current license.
- Minimum of two year of nursing experience as a R. N.
- Ability to relate positively and favorably with clients and staff.
- Demonstrate knowledge and skills necessary to provide care primarily to the geriatric population, and to a lesser degree, the pediatric and adult population.
- Professional demeanor and appearance.
- Excellent oral and written skills along with documentation skill.
- Demonstrate organizational and leadership skills.
Pay: $25.00 - $50.00 per hour
