*Job Summary* Paloma Springs Healthcare is seeking a dedicated and compassionate Nurse LPN/RN to join our healthcare team. The ideal candidate will provide high-quality patient care across diverse clinical settings, including acute, chronic, and long-term care environments. Experience with electronic health records (EHR/Epic systems), infection control, and specialized care such as dementia, Alzheimer's, or pediatric care is highly valued. - Conduct comprehensive patient assessments, including vital signs, physical examinations, and medical history documentation.
- Administer medications and injections accurately following physician orders and established protocols.
- Perform specialized procedures such as IV infusion, catheterization, phlebotomy, wound suturing, and sterile processing.
- Manage airway interventions, ventilator support, and advanced life support measures as needed.
- Assist with diagnostic evaluations including laboratory specimen collection, medical imaging coordination, and diagnostic testing such as spinal taps or cardiac catheterization.
- Provide patient education on medication management, disease processes (e.g., dementia, infectious diseases), nutrition, and post-discharge care plans.
- Document all patient interactions thoroughly within EMR/EHR systems like Cerner or Athenahealth while maintaining HIPAA compliance.
- Collaborate with physicians and healthcare teams to develop individualized care plans for patients in settings such as ICU, Emergency Room, hospice, or outpatient clinics.
- Ensure infection control standards are upheld through aseptic technique and proper sterilization procedures.
- Participate in discharge planning and case management activities to facilitate smooth transitions of care across settings including home health or post-acute environments.
*Requirements*
- Valid LPN or RN license in the state of practice.
- Proven experience in diverse clinical settings such as ICU (Level I/II Trauma Center), emergency medicine, geriatrics (including Alzheimer's and dementia care), pediatrics (including neonatal ventilator management), or home & community health.
- Proficiency with EMR/EHR systems such as Epic, Cerner, eClinicalWorks, or Athenahealth.
- Strong knowledge of medical terminology, anatomy & physiology, CPT/ICD coding (ICD-9/10), and diagnostic evaluation techniques.
- Ability to perform advanced procedures including IV insertion/infusion, venipuncture, tube feeding management, and airway management.
- Experience working with special populations such as patients with developmental disabilities or behavioral health needs is preferred.
- Excellent patient assessment skills combined with compassion for delivering holistic care across acute to chronic conditions.
- Ability to work independently while collaborating effectively within multidisciplinary teams.
- Knowledge of infection control protocols and sterile techniques essential for maintaining safety standards.
- Must pass background checks and comply with all licensing requirements; BLS/CPR certification required.
