The Crossover LVN/LPN is central to creating a strong, seamless patient and member experience within our clinic. This role actively manages patient care by coordinating with specialists and their facilities, guiding patients and members through the healthcare process, and demonstrating in-depth knowledge of self-insured employer health insurance, including copays, co-insurance, and related benefits. As an important member of our clinic team, you will assist with patient check-ins and ensure that our fast-paced, rapidly changing environment operates smoothly while upholding our Crossover’s values.
You will play a key role in facilitating the patient and member experience, ensuring that primary care is delivered as the patient-centered home, following evidence-based medicine guidelines.

You will play a key role in facilitating the patient and member experience, ensuring that primary care is delivered as the patient-centered home, following evidence-based medicine guidelines.
Clinical Responsibilities
Delivers patient care, practicing under the oversight of an RN or clinic supervisor
Collaborates with patient and care team to facilitate a patient-centered medical home
Provides evidence-based nursing care at the highest community standard
Take medical histories, measure patient vitals, and other pertinent information to assist the clinician with care of the patient
Coordinates and connects the patient with specialists and their facilities
Provides phlebotomy services and immunization services
Performs other duties as assigned
Member Service Responsibilities
Welcomes each member and handles all member interactions with the highest level of hospitality and professionalism.
Demonstrates a commitment to quality, delivering uncompromised service and outcomes.
Accommodates special requests for member support whenever possible and helps to promptly resolve member questions.
Assists with assigned projects and special provider or patient requests.
Assists with the member check-in process, including a basic understanding of benefit eligibility files, check-in technology and overall clinic workflows within the context of care delivery.
Processes transactions required to begin or complete the visit.
Demonstrates an in-depth knowledge of employer-sponsored health insurance, including copays, co-insurance, and related benefit specific requirements, in order to support members.
Engages with members by answering calls, scheduling appointments, responding to emails, and following up with member needs.
Collaborates with providers and staff members to deliver an exceptional seamless patient-centered care experience.
Performs other duties as assigned
Required Qualifications
Licensed Vocational Nurse from an accredited school with a minimum of 3 years of clinical experience
Current BLS (Basic Life Support) certification
Preferred Qualifications
Experience in a fast-paced and rapidly changing environment
Demonstrated strong work ethic and takes pride in a detailed oriented approach to work
Highly organized and enjoys managing projects, big and small, to completion
Creative, flexible, and navigates through obstacles
Excellent follow-through with execution skills
Able to handle ambiguity and solving issues
Physical Job Requirements
Requires standing, walking and sitting for extended amounts of time.
Occasionally lift and carry items weighing up to 50 lbs.
Manual and finger dexterity and hand-eye coordination
Includes full range of body motion including potential of handling and lifting patients.
Requires corrected vision, hearing and speech within normal ranges. Must be able to effectively communicate with patients and team members.
The base pay range for this position is $37.35 to $49 per hour. Pay range may vary depending on work location, applicable knowledge, skills, and experience. This position may be eligible for an annual bonus opportunity and comprehensive benefits package that includes Medical Insurance, Dental Insurance, Vision Insurance, Short- and Long-Term Disability, Life Insurance, Paid Time Off and 401K.
