We're seeking a dedicated and compassionate LPN to join our dynamic healthcare team. As an LPN at Hamilton Grove, you'll be responsible for providing basic nursing care to our patients, including administering medications, performing vital signs, and assisting with activities of daily living.

You'll also work closely with our interdisciplinary team to develop and implement personalized care plans, ensuring that our patients receive the highest level of care.

Responsibilities

  • Provide basic nursing care to patients, including administering medications, performing vital signs, and assisting with activities of daily living
  • Develop and implement personalized care plans in collaboration with the healthcare team
  • Conduct routine vital sign checks and document patient information accurately
  • Assist with patient transfers and ambulation as needed
  • Provide education and support to patients and their families
  • Participate in quality improvement initiatives to ensure high-quality patient care.
