*Duties*

• Oversee and manage health information management for a primary care facility
• Utilize electronic health record systems such as Athenahealth, eClinicalWorks, and Cerner to maintain accurate and up-to-date patient records
• Ensure compliance with industry standards and regulations related to health information management
• Collaborate with healthcare providers to collect and analyze data for quality improvement initiatives, such as HEDIS measures
• Coordinate discharge planning for patients transitioning from hospital to nursing home or hospice care
• Conduct assessments and evaluations to determine appropriate levels of care for patients
• Assist in the development and implementation of care plans for patients
• Provide education and training to staff on documentation requirements and best practices
