Responsibilities

  • Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using patient transfer methods and equipment like Hoyer lifts.

  • Monitor and document patient conditions through patient observation, vital signs measurement, and medical documentation to ensure accurate records in EMR (Electronic Medical Records) or EHR systems.
  • Administer medications as prescribed, including medication administration and catheter care, following home healthcare regulations and safety protocols.
  • Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care techniques and behavior management strategies.
  • Support patients during end-of-life care or hospice situations with compassion and respect for their dignity.
  • Maintain patient confidentiality in accordance with HIPAA regulations while communicating effectively with clients, families, and healthcare teams.
  • Perform light housekeeping duties such as cleaning, laundry, meal preparation, and ensuring a safe environment conducive to client well-being.

Skills

  • Proven experience working with geriatric patients or individuals with developmental disabilities in settings such as nursing homes or home health agencies.
  • Strong knowledge of medical terminology, medical records management, and home health care assistance procedures.
  • Skilled in patient transfer methods, patient personal care assistance, and using equipment like Hoyer lifts safely.
  • Ability to perform CPR and First Aid confidently; current certification required.
  • Familiarity with EMR/EHR systems for accurate medical documentation and record keeping.
  • Excellent client communication skills with a caring approach to caregiving and client engagement.
  • Physical ability to perform heavy lifting when necessary and work flexible hours in live-in employment situations if required.
