Summary
TheraCare Home Health is seeking a compassionate and experienced Registered Nurse (RN) to provide high-quality, patient-centered care in the home setting. In this role, you will manage a caseload of patients, complete comprehensive assessments, and coordinate care across the interdisciplinary team. You will play a key role in supporting patients throughout their home health episode while promoting independence, safety, and positive clinical outcomes.
Qualifications
Graduate of an accredited Diploma, Associate, or Bachelor’s degree in Nursing Current Registered Nurse (RN) license in the state of Texas Minimum 2 years of RN clinical experience; home health experience preferred Strong knowledge of home health regulations, OASIS documentation, and care planning Experience completing Start of Care (SOC), Recertifications, and Resumption of Care visits preferred Ability to manage a patient caseload and work independently in a field-based environment Experience coordinating care with interdisciplinary teams (PT, OT, ST, MS
