About this role
We are looking for an experienced Registered Nurse to serve as Director of Patient Care Services (DPCS) for a Medicare-certified home health agency located in Sherman Oaks.
This is a hands-on clinical management position overseeing the quality and coordination of patient care, field nurses and therapists, clinical documentation, physician orders, OASIS/QA, and regulatory compliance.
You must be a California Registered Nurse to qualify for this position. This is a regulatory requirement.
This is approximately a 2/3-time position, generally 26–28 hours per week , rather than a traditional 40-hour-per-week management role.

You will not be managing the clinical operation alone. The agency has another experienced RN who will assist with clinical oversight, case management, documentation review, and day-to-day clinical issues. We also have administrative support so the DPCS can focus primarily on clinical management rather than routine clerical work.
The DPCS will work closely with the owner, who focuses on the business, technology, marketing, and growth of the company.
We are a technology-focused organization and are continually improving our EMR, clinical workflows, data systems, and quality-management processes. We are looking for someone who is comfortable learning new systems and interested in helping build an efficient, high-quality home health operation.
Responsibilities
- Oversee the agency's clinical operations and delivery of patient care
- Supervise and support RN/LVN case managers and field clinicians
- Provide clinical oversight of nurses, therapists, and other patient-care staff
- Review SOC, ROC, recertification, transfer, and discharge documentation
- Oversee plans of care, physician orders, and clinical coordination
- Monitor OASIS documentation and quality
- Assist with hospitalization reduction, patient outcomes, and Medicare Star Rating improvement
- Participate in QAPI and clinical performance improvement
- Handle clinical escalations, incidents, complaints, and high-risk patients
- Ensure compliance with Medicare, California Department of Public Health, and home health regulations
- Help develop efficient clinical processes as the agency grows
Qualifications
- Active California RN license — required
- Minimum 2 years of clinical nursing experience
- Home health experience strongly preferred
- Prior supervisory, case-management, DON, DPCS, clinical-manager, or similar leadership experience preferred
- Strong organizational and communication skills
- Comfortable supervising and holding staff accountable for quality and timely documentation
- Comfortable learning and using EMR and other technology systems
- Able to work independently while collaborating with another experienced RN and agency ownership
Schedule
Approximately 26–28 hours per week (2/3 FTE) .
The schedule can have some flexibility, but this is primarily an in-person position in Sherman Oaks .
Compensation
Approximately $85,000–$95,000 per year , depending on experience and qualifications, for the approximately 2/3-time schedule.
There is an opportunity for increased responsibility, hours, and compensation as the agency grows.
