What You'll Do
- Provide skilled nursing care in the patient's home
- Administer medications and treatments as ordered
- Monitor the patient's condition and report changes promptly
- Assist with prescribed feeding and other skilled nursing tasks, as needed
- Maintain accurate and timely clinical documentation
- Follow the established plan of care
- Communicate with the family and BrightStar Care clinical team
What We're Looking For
Active New York State Licensed Practical Nurse (LPN) License* At least 1 year of nursing experience*
- Home care or one-on-one nursing experience preferred
- Strong clinical judgment and communication skills
- Dependable, professional, and patient-focused
- Comfortable working independently in a home care setting
